2. WHAT WE COLLECT
Through the contact form: your full name, phone number, email address (optional), the topic of your request, and the message you write.
Please do not include a diagnosis, medical record number, or other sensitive health details in this form.
Automatically: the page you came from (its address only, not any search terms or identifiers it may have carried) and the date and time of submission.
We also keep an internal record of which authorized staff member accessed, changed, or deleted your request, and when — for accountability, not for contacting you. If your name, phone number, email address or message is corrected, this internal record notes only that the field changed; it does not store the corrected value itself. This internal access record is kept for one year.
3. WHY WE COLLECT IT, AND OUR LEGAL BASIS
We use this information to respond to your enquiry, arrange the help you asked for, and keep an internal record of correspondence with patients and visitors.
To the extent that cookie identifiers, Internet Protocol addresses, browser or device information, website logs, or similar technical information constitute personal data, Republika Ixtisoslashtirilgan Onkologiya va Radiologiya Ilmiy-Amaliy Tibbiyot Markazi processes such information in accordance with the legislation of the Republic of Uzbekistan, including the Law of the Republic of Uzbekistan No. ZRU-547 "On Personal Data", as amended.

Where you voluntarily provide information concerning your health or medical records, that information is processed only with your separate, explicit electronic consent, given through its own checkbox on the form, for the purpose of reviewing your request and arranging appropriate medical services. You may withdraw your consent at any time; withdrawal does not affect the lawfulness of processing carried out beforehand. Optional analytics cookies are used only where you have given consent through the cookie banner or the "Cookie preferences" link in the website footer, and are used to measure website traffic, understand how visitors use the website, identify technical problems, and improve the website's content and performance.
The lawful and legitimate interests of the Cancer Centre. Limited technical information and strictly necessary cookies may be processed where required to operate, maintain, protect, and ensure the security and reliability of the website, provided that such processing does not violate the rights and legitimate interests of website visitors.

4. HOW LONG WE KEEP IT
Contact-form records are kept for three months, after which they become eligible for deletion.
5. WHO CAN SEE IT
Authorized hospital staff who triage and respond to enquiries from patients and visitors. We do not sell this information. There are no other recipients or processors of contact-form data. Website analytics are covered separately in our Cookie Policy.

Subject to the conditions and limitations established by the legislation of the Republic of Uzbekistan, you have the right to:

The Cancer Centre does not use website analytics to make decisions that produce legal or similarly significant consequences for individual visitors.

If you consider that the Cancer Centre has not adequately addressed your complaint, you may apply directly to the authorised state body in the field of personal data of the Republic of Uzbekistan, through the official State Register of Personal Data Databases portal (currently operated by the Department of Migration and Personalization under the Ministry of Internal Affairs), or to a competent court of the Republic of Uzbekistan.
8. HOW WE PROTECT YOUR INFORMATION
The Cancer Centre applies legal, organisational, and technical measures appropriate to the nature and limited volume of personal data processed through this website. These measures are intended to protect information against unauthorised or unlawful access, processing, alteration, disclosure, loss, or destruction.
The measures currently in place for this website include:
data transmitted through this website protected using HTTPS/TLS;
individual accounts for each authorised member of staff, with access to the website administration system restricted to those accounts;
a record of which authorised staff member accessed, changed, or deleted an enquiry, and when;
defined retention periods, after which contact-form records and the internal access record are deleted;
loading website analytics only after the visitor has given consent, and collecting no more information than the analytics service needs;
advertising personalisation and advertising-related analytics features disabled.
The Cancer Centre does not intentionally enter or transmit patient names, telephone numbers, identification numbers, medical-record numbers, diagnosis information, genetic information, or other medical information through the website analytics system.
Although the Cancer Centre takes reasonable measures to protect information, no method of transmission over the Internet and no method of electronic storage can be guaranteed to be completely secure.

Every contact-form submission receives a short reference number, shown on the confirmation screen, so you can quote it if you follow up. We aim to respond within 30 days.
