Medical records checklist
What to send, and what can wait. You do not need everything on this list to write to us — the first two lines are enough to begin.
Essential for any review
- The doctor's summary or referral letter.
- The pathology (histology) report — the laboratory report on the tissue sample.
- Written imaging reports: CT, MRI, PET, ultrasound or X-ray.
- A list of current medicines with doses.
Very helpful if you have them
- The imaging files themselves, on disc or drive — not only the report.
- Pathology slides or paraffin blocks, so our laboratory can re-examine the tissue.
- Recent blood tests, including blood count and liver and kidney function.
- Operation reports and discharge summaries from previous treatment.
- Details of chemotherapy or radiotherapy already given: drug names, doses, dates, number of sessions.
Only when we ask for it
- A copy of the passport — requested for admission or visa-support documents, not for the clinical review.
- Insurance documents, if an insurer is paying.
- A signed authorisation, if a relative or organisation is writing on the patient's behalf.
How to send them
Send documents to the address in the bar at the top of this page. For imaging files or anything large, write to the same address and the coordinator will send you a secure way to transfer them. Please do not send medical records through the general website contact form or through social-media messaging.
